May 9, 2012 any Credit Card, Debit Card or any method of payment other
than by Cash, Check or Wire Transfer shall incur a 4% Processing Charge.
If you would like information on wiring your payments, it can be emailed
to you upon request, there is no charge for wiring us a payment. |
| Mountain Valley Express adds a Notification Fee |

| Clean Air Compliance |
| Due to the California Clean Air Resources Board [CARB] regulations, Mountain Valley Express needs to increase our Clean Air Compliance Fee to $2.95 per shipment effective July 1, 2012. For further information please review Item 647 in our MTVL 300 Rules Tariff. |
Fixed Fuel Surcharge Fee |

pallet rate with a fixed FSC will be increased to a flat 20% FSC effective
June 1, 2012. |
